◯◯◯◯ Inc. (“we”) handles the personal information obtained in providing Wellness Director Azabu (the “Service”) in compliance with the Act on the Protection of Personal Information and other applicable laws, and in accordance with the policy below.
1. Information we obtain
In providing the Service we may obtain the following.
- Basic details: name, contact details (email, telephone) and address
- Whatever you enter in the enquiry form (provided via UTAGE)
- Information concerning your family and parents, obtained with their consent
- Sleep, activity and heart-rate data recorded by a wearable device
- Results of screenings, check-ups and examinations, and records of appointments
- Records of meals and exercise, and other information concerning your condition
- The content of your requests and the history of arrangements made
2. Special care-required personal information
Screening results, records of medical appointments and other information concerning health constitute “special care-required personal information” under the Act on the Protection of Personal Information. Data recorded by wearable devices is handled with equivalent care.
Before obtaining such information we explain the purpose, how it is stored and the scope of any sharing, and we obtain your explicit consent. We do not use it beyond the scope of that consent.
Where such information concerns your family or parents, it is obtained with their own consent.
3. Purposes of use
We use the information obtained for the following purposes.
- Providing the Service, responding to requests and carrying out arrangements
- Understanding your condition and proposing and arranging training, meals and sleep environment
- Holding and advising on the schedule of screenings and appointments
- Carrying out arrangements for your family and parents
- Administering the agreement and invoicing fees and costs
- Maintaining and improving the quality of the Service
- Meeting obligations under applicable law
4. Disclosure to third parties
We do not provide personal information to third parties without your prior consent, except:
- where required by law; or
- where necessary to protect a person’s life, body or property and consent is difficult to obtain.
By the nature of the Service, carrying out a request may require passing necessary information to medical institutions, specialists (dietitians, physiotherapists, trainers, chefs and others) or external providers. In such cases we share only the minimum required, having explained the scope and obtained your consent.
Recipients are bound by confidentiality obligations under contract.
5. Storage and security
We take necessary and appropriate measures to prevent unauthorised access, loss, destruction, alteration and leakage of personal information.
Access to special care-required personal information is limited to your director and those whose work requires it, and a record of handling is kept.
Our officers, staff and contractors are bound by confidentiality obligations.
6. Retention, and deletion on departure
We retain personal information only for as long as is necessary to achieve the purposes of use.
On departure we delete the data and records we hold at your request, other than anything we are required by law to retain, and confirm when deletion is complete.
7. Disclosure, correction and suspension of use
We respond appropriately, in accordance with applicable law, to requests to disclose retained personal data, to correct, add to or delete it, to suspend or erase its use, or to stop its provision to third parties. Please make such requests through the contact point on this site.
8. The enquiry form
Enquiries on this site are handled through a form provided by an external service (UTAGE). The terms and privacy policy of that service also apply to the information you enter there.
9. Analytics
We may use analytics tools to understand how this site is used. These collect information using cookies and similar technologies but do not identify you personally. Cookies can be disabled in your browser settings.
10. Changes to this policy
We may revise this policy in response to changes in law or in the Service. Any revised version will be published on this page.
